Item System

Items in Dandy’s World are single-use tools that help you survive, move, and extract more safely. You carry them in a limited inventory, then consume or activate them at the right time to gain a short-term advantage. Items are separate from trinkets. Trinkets are permanent loadout choices for the entire run, while items are picked up on the floor and disappear once used.

Most items are found in containers, on shelves, or scattered around machines, with a few coming from Dandy’s Shop or special events. When you pick an item up, it fills a slot in your inventory. If your inventory is full, you must swap or leave something behind.

Items broadly fall into the categories in this chapter. Each category has its own typical use cases and risks. Strong play is not just about knowing what an item does, but when to use it and what to leave on the ground.

Important rule: Because inventory slots are limited, every item you pick is a trade-off. Never carry something that you do not expect to use in the next one or two floors.

Healing Items

Healing items restore hearts or help you recover after mistakes. They are your insurance against misplays, bad spawns, and unexpected Twisteds.

Common healing items usually restore a fraction of a heart or a single heart. Rarer ones can restore multiple hearts or even fully heal you. Some healing items may have a short use time, during which you are vulnerable, so you must pick safe moments to heal, such as when you are out of line of sight or in a known safe corner.

There are three key ideas for healing items. First, they are most powerful when you use them before a dangerous floor, not after you are already dead. Second, overhealing is wasteful. If a healing item restores 2 hearts and you are missing only 1, you effectively waste 1 heart of value. Third, in multiplayer, healing items can save an entire run when used to keep key roles alive, especially extractors and experienced distractors.

Healing rule: Aim to heal when you are one mistake away from dying, not at full health and not when you are already one hit from death during an active chase.

Stamina Items

Stamina items recover your stamina bar or reduce stamina costs for a short duration. They are vital for escape, kiting, and repositioning. Good stamina management often decides whether you escape a Twisted or fall.

Basic stamina items give a burst of stamina, letting you sprint longer or recover from a long chase. Some stronger items temporarily increase your maximum stamina or make stamina drain slower. Any item that influences stamina becomes extremely valuable during Panic Mode, during blackouts, or on higher floors where Twisteds are faster and more punishing.

Stamina items are usually not worth using just to move between safe parts of the map. Their main value is in emergencies. Save them for when a chase begins in a bad location, when you need to cross open ground, or when you misjudge a route and risk running out of stamina with a Twisted near you.

Escape rule: Never pop a stamina item at the very end of your stamina bar. Use it slightly earlier so you do not get caught during the use animation.

Extraction Items

Extraction items help you complete machines faster, safer, or with fewer skill checks. They are the main tools for speeding up runs and are extremely valuable when your goal is to push floors, farm research, or finish daily tasks efficiently.

Some extraction items boost your extraction speed on a single machine. Others may affect all machines for a short time or reduce skill check difficulty. A few items can instantly complete part of a machine’s progress bar. Strong extraction items become more powerful the higher you go, because each second saved reduces the time you are exposed to Twisteds.

Extraction items are strongest when used on machines in dangerous spots or late in a floor, when multiple Twisteds are active and any extra time you spend on a machine is risky. They also let weaker extraction toons keep up with stronger teammates. If your toon has low extraction stats, pairing them with powerful extraction items can make you viable even on higher floors.

Extraction rule: Use extraction items on high-risk or late-floor machines, not on the first safe machine you touch.

Utility Items

Utility items do not directly heal, restore stamina, or speed extraction, but they change how you interact with Twisteds or the map. They are the most flexible category and frequently decide whether a bad situation becomes a wipe or a recovery.

Typical utility effects include creating noise to distract Twisteds, blocking or slowing enemies, revealing parts of the map, improving stealth for a short time, or allowing quick repositioning. Some utility items are proactive, such as items you set up before a chase to create safer loops. Others are reactive, used in emergencies to break line of sight or reset Twisted aggro.

Because utility items are so varied, their value depends heavily on your role. Distractors get enormous value from noise and blocking tools, while extractors can benefit more from stealth and quick repositioning tools that let them stay on machines longer.

Utility rule: A “wasted” utility item that prevents a death is never truly wasted. Use them early in a bad situation, not after it is already out of control.

Rare Items

Rare items are stronger versions of common items or unique tools with powerful effects, such as huge heals, massive stamina boosts, large extraction jumps, or strong crowd-control style utility. They often have longer use times, louder sound cues, or stricter conditions, but they can completely turn a run around when used correctly.

You will not see rare items all the time, so treat them like limited power spikes. Decide in advance how you plan to use them. For example, you might hold a rare heal specifically for the floor before a known spike in difficulty, or you might keep a strong extraction item for the final machine of a dangerous floor.

Some rare items are so strong that carrying them justifies dropping two weaker items. Items that combine multiple roles, like healing plus stamina or stamina plus stealth, are especially valuable, because they solve several problems at once.

Priority rule: In most cases, one good rare item is worth more than several weak commons. Do not clutter your inventory with low-impact items if it means leaving a rare one behind.

When to Use Each Item

Using items at the correct time is as important as picking the right ones. The basic timing patterns for each type work like this.

Healing items are best used between fights, chases, or machines, when you are safely hidden and not at immediate risk. Use them when you are missing enough health to get full value, but before entering a dangerous area or high-floor machine. If you expect a lethal floor or a rough Twisted combination next, go into it with as many hearts as you can.

Stamina items belong to chase situations. Use them when you realize your route will keep you exposed for longer than your natural stamina allows. For example, when you need to reach a known safe loop across a large open room, or when a Twisted cuts off your usual route, forcing you into a longer path. Avoid using stamina items just to move slightly faster on an empty floor.

Extraction items should be used on machines that either feel dangerous due to location, or are late in the floor when every second counts. If multiple Twisteds are active and your team has drawn a lot of aggro, an extraction item can finish a machine before a new threat arrives. They are often wasted on the first easy machine of a floor.

Utility items are the most context dependent. Use noise or distraction items before your extractor starts an exposed machine, stealth tools before crossing a risky hallway, and emergency tools at the first hint that a situation is getting bad. If you see two Twisteds converging on your route, that is already the time to consider using a utility item, not when they are on top of you.

Inventory Management

Inventory management is about deciding what to carry now in order to survive and progress over the next few floors. Space is limited, so you must constantly evaluate your items in light of the current run: your hearts, your stamina comfort, your role, and the twisteds and floor you are facing.

Think of each slot as a job. One slot might be dedicated to healing, one to emergency stamina or escape, and one to either extraction or utility, depending on your role. If you already have reliable healing, picking a second weak heal may be worse than holding a strong stamina or escape tool.

A simple way to evaluate an item is to ask: “Can I imagine a realistic situation in the next one or two floors where this item is crucial?” If the answer is no, drop it. The earlier you start making disciplined inventory decisions, the better your runs will go.

A useful way to reason about inventory is to think in terms of expected value. Suppose there is an item A that might save you in one rare scenario and an item B that is useful in many common scenarios. Item B has higher expected value and usually deserves the slot, even if A looks more dramatic.

Inventory rule: Regularly re-evaluate every item when you find something new. Do not keep an item just because you picked it up several rooms ago.

Best Items to Prioritize

For beginners, the best items to prioritize are the ones that are simple, reliable, and clearly useful in many situations. Healing items that restore a full heart or more are top priority, especially if your team does not have strong survival trinkets or support toons.

Next in priority are stamina and escape items, because surviving a chase is more important than slightly faster extraction. If you often die while trying to run away, focus on always holding at least one item that helps you escape, such as a strong stamina boost or an emergency repositioning tool.

Extraction items are a priority when you are confident in your ability to survive but want to reach higher floors faster. On coordinated teams, at least one player should prioritize strong extraction items, while another focuses on utility and survival. For solo play, try to carry at least one survival item and one extraction item, then fill the last slot with whatever you are personally weakest at.

Utility items climb in priority as you become more experienced. Beginners tend to underestimate them, but players who know their maps and Twisted behavior can turn a single utility item into multiple saved hearts, finished machines, or rescued teammates.

In summary, early on, favor healing and escape. As your skill and comfort grow, shift more of your inventory toward extraction and utility to speed up runs and push to higher floors.
